What Drivers Should Bring
- Please bring your medications, any supporting medical documents, your eyewear and hearing aids. You don't need to bring DOT physical forms — we have everything else needed for your exam.

More About the DOT Physical Exam
To drive a commercial motor vehicle (CMV), you are required to pass a DOT medical examination. This exam evaluates your overall physical health.
The medical examiner conducting your DOT physical will have you fill out a healthcare history form covering your medical background and current health status.
Your DOT physical includes a thorough examination of your hearing, vision and blood pressure. A urinalysis is required and will be done. Other in-office tests, such as an electrocardiogram, may also be performed.
Every test is meant to confirm that your health allows you to safely operate a commercial vehicle.
After you pass the Department of Transportation physical, you'll receive a medical certificate that lets you drive a commercial vehicle for two years — unless an existing physical or psychological condition requires more frequent testing.
